Eligibility & Requirements
The selected therapist must be licensed.
You must be currently seeing the therapist or have an appointment prior to applying.
The applicant must be able to pay for the sessions upfront and be reimbursed on a monthly basis.

Your counselor’s name and the counseling center
You must have a counselor identified in order to apply. If you haven’t found one yet, we’d be happy to recommend a trusted provider from our vetted network.
A letter from your counselor
This letter should confirm your engagement in therapy and briefly describe the therapeutic goals or areas of focus.
The cost per session
Please indicate whether you are using insurance, private pay, or a combination.
Your family's story and need
This includes a brief summary of your adoption or foster care journey and an explanation of your current financial need and how this grant would help support your family's well-being
